FORM W7 – APPLICATION CHECKLIST
Information for completion of the ITIN or Form W7 Application.
What is an ITIN?
An Individual Taxpayer Identification Number (ITIN) is a 9-digit number [Usually this number starts with digit 9] the Internal Revenue Service ITIN operation office issues this number to foreign persons filing a tax return who are not U.S. citizens and who do not have or are not eligible for a Social Security number. You can only use your ITIN for tax reporting purposes.
Please note that if you have a Social Security Number [SSN], you are not eligible to apply for ITIN.
Checklist for the ITIN application preparation
Before submitting your Form W-7 and the attached documentation – This checklist would be a helpful option
- Verify that you are completing the most current version of the Form W-7.
- Verify that the entries on Form W-7 do not conflict with the supporting documentation presented.
- Ensure the entries are typewritten or printed clearly and legibly in ink.
- Ensure that you checked the appropriate box indicating the “Reason you are Submitting Form W-7
- Verify that the name on Line 1 matches the name on the supporting documentation.
- Enter your complete mailing address on line 2.
- Enter your permanent foreign address on Line 3, or if you have relocated to the United States [if applicable]. Foreign address is mandatory for applicants outside of the United States.
- Verify that the country of birth is present on Line 4, and that the date of birth is entered in a month/day/year (MM/DD/YYYY) format.
- Place an “X” in the appropriate “gender” box on line 5.
- Ensure that you entered your complete visa information (if applicable) on line 6c.
- Check that you entered your complete date of entry into the United States (if applicable) in a month/day/year (MM/DD/YYYY) format.
- Check that all Form W-7 applications have a signature that matches the signature on the supporting documentation, or in the case of a delegate, reflects the delegate’s relationship to the applicant.
- Enter the date of the application in a month/day/year (MM/DD/YYYY) format.
Submit: Your completed valid U.S. Federal income tax return (if applicable) or documents substantiating that you meet an exception to the tax return filing requirement.
