How to respond to CP54B Notice?

The CP54B notice is usually sent in with a response form for your completion to confirm the accuracy of the data before the release of the refund cheque to avoid the cheque being sent to the wrong person or address.

  • Section 1 – You need to check the appropriate box of the 3 options provided to confirm if the ITIN number is correct and the name is wrong, the Name is correct, and the ITIN number is wrong or the option to confirm if the name and ITIN number is correct.
  • Section 2 – You need to write your ITIN number and name exactly as received on your CP565 ITIN approval letter if you received one or provide the ITIN number and correct name.
  • Section 3 – Write in detail any discrepancies in name spelling or ITIN number.
  • Section 4 – You need to provide your name and mailing address confirmation.
  • Section 5 – You need to sign on the section
  • Section 6 – In this section IRS will provide the mailing address you need to send the response form.

Important tip – It is recommended to enclose the CP565 ITIN approval letter, passport copy, and 1040NR pages 1 and 2 to the response form to provide proof of evidence
